   STEP 3: Employee Selection
         
   
         
Employee 1 receives packet at meeting. Employee signs receipt page immediately and hands back. Takes enrollment package and returns completed form within 30 days to processing mailroom. Employee 2 receives packet in the mail. Delivery is validated through proof of service. They return completed form within 30 days to processing mailroom. Employee 3 does not return completed form.
